The Coordinated Case Management for Youth and Families project is a rapid rehousing program for transition age youth. The project is a part of the Journey Home OKC housing first initiative and is a collaborative effort between Be The Change, the Homeless Alliance and additional partners that provide support services for vulnerable and at-risk youth.
